What version? If 7.1, make sure you're patched past 7.1.14. There was a bug fix where
under heavy inserts it was reporting that error and has been changed to an internal error.

What does it mean if a transaction fails with "rowid already allocated" ?

Should i just re-commit the transaction to try again ?
