Did you try the suggestion in the MySQL manual :

Installing ActiveState Perl on Win32
------------------------------------

To install the *MySQL* `DBD' module with ActiveState Perl on Win32, you
should do the following:

   * Open a DOS shell.

   * If required, set the HTTP_proxy variable. For example, you might
     try: `set HTTP_proxy=my.proxy.com:3128'

   * Start the PPM program: `C:\perl\bin\ppm.pl'

   * If you have not already done so, install `DBI': `install DBI'

   * If this succeeds, install `DBD::mysql:'
     `http://www.mysql.com/Contrib/ppd/DBD-mysql.ppd'

If you can't get the above to work, you should instead install the
MyODBC driver and connect to *MySQL* server through ODBC.

     use DBI;
     $dbh= DBI->connect("DBI:ODBC:$dsn","$user","$password") ||
       die "Got error $DBI::errstr when connecting to $dsn\n";
