Well, I'm pondering whether it's possible to get rid of all those macros,
and still maintain the power of sql_create_*()... but I'm not even close to a
proposal yet, let alone a patch.
It's probably fine as is, but the current code is a little dense and hard
to figure out, and relies on external code to generate it.
