Is this a 'fresh' install of MySQL-4.1.3, or an upgrade install where
you've replaced the binaries over some 'older' install of MySQL-4.1.3?

Are you running with '--old-passwords', or have you kept 'old' grant
tables around?

Have you upgraded the grant tables as well as the passwords?
