czech case sensitive collate for win1250 and latin2 encodings

Hello

This patch contains "true" case insensiteve collates for czech latin2 and 
cp1250 encodings. I respect all Alexander Barkov comments.

Current latin2_czech_cs is inconsistent, LIKE operator is case insensitive, 
but I have not courage change it. There are lot of application which use it. 
It's reason why I can share "like" functionality. I don't wond duplicate 
code and tables.
