Have you executed the mysql_fix_privilege_tables script after updating 
the server?

This script has to be executed after each server update until it passes 
without errors. This should fix the crash you are seeing.

Also, please seriously consider to update to a more recent version of 
the MySQL Server. A lot of issues have been fixed since 5.0.16.
